Once the forms are completed, please click on the submit button on the upper left hand corner. Please read the information on the First Visit page, for additional information regarding the Financial & Insurance, and Office Cancellation policies.

We highly advise that you submit the completed forms online to expedite your check-in process when you arrive for your appointment.
